Seven Hills is a Conflict-Free Case Management Agency
Conflict-Free Case Management (CFCM) in Rhode Island is a required Medicaid service for individuals receiving Long Term Services and Supports (LTSS), especially those in Home and Community Based Services (HCBS) programs. It ensures that the people who help individuals plan their services are independent from the agencies that provide those services, reducing conflicts of interest and improving quality of care.
Purpose of CFCM is to ensure individuals access the best services
CFCM exists to ensure:
- Person-centered planning where the individual's goals, preferences, and voice lead the process.
- Unbiased guidance, since case managers are separate from service providers.
- Fair access to services across the developmental disability and LTSS systems.
Who does CFCM serve?
CFCM supports:
- Adults receiving Medicaid LTSS, including those with intellectual and developmental disabilities (I/DD) and elders/adults with disabilities (EAD).
What do CFCM Case Managers do?
CFCM case managers help individuals:
- Lead their Person-Centered Planning (PCP) meetings.
- Identify goals, resources, and supports—both inside and outside the DD system.
- Write and update their annual PCP and ensure documentation is complete.
- Explore new opportunities, activities, and community connections.
- Monitor services, check in regularly, and ensure satisfaction and safety.
