Therapeutic Trackers (also referred to as Therapeutic Mentors) support youth who are experiencing emotional/behavioral issues in their home, school, or community. We offer structured, one-to-one strength-based support services between a tracker/mentor and a youth, for the purpose of providing a meaningful relationship, positive role modeling, and an advocate for the youth.

Population Served

  • Youth ages 5-21 years all ages in a public school system
  • Youth struggling with issues include anger management, social anxiety, depression, low self-esteem, lack of social skills, and others

Overview of Services

YOU Inc.’s Therapeutic Tracking/Mentoring program is offered within the framework of the Massachusetts Children’s Behavioral Health Initiative (CBHI), and uses a strengths-based approach in a Wraparound model. In coordination with other behavioral health providers, Therapeutic Trackers/Mentors assess a child’s presenting needs and skills deficits. Building upon the child’s strengths and interests, the Tracker/Mentor supports, coaches, and trains the youth to improve age-appropriate skills in the areas of social skills, problem-solving, daily living, conflict resolution, behavior management and self-management. The Tracker/Mentor assists the child in practicing these skills in real-world social interactions and contexts, while also modeling appropriate behavior and offering the opportunity to process and discuss presenting issues therapeutically.

Treatment team meetings occur on at least a monthly basis with all service providers in order to assess, as a group, the child and family’s progress toward identified treatment goals. To support the Therapeutic Tracking program’s goal to be a family-driven service, we offer appointment times that are flexible and most convenient for the children and families with which we work. Therapeutic Tracking services are available seven days per week from the hours of 8 am to 8 pm, with the exception of major holidays.

Referral Information    

In order to be eligible for Therapeutic Tracking, a youth must be under the age of 21 with Medicaid insurance, and must have at least one of three “clinical hub” services in place [i.e. Intensive Care Coordination (ICC), In-Home Therapy (IHT), or Outpatient Therapy].
