From Hospital to Home

Our goal is your goal: to transition your child home as quickly as possible.

Seven Hills Pediatric Center provides short-term post-hospitalization care for children, regardless of cognitive level, who require 24-hour medical support and rehabilitative services in the transition from hospital to home. Our professional staff specializes in pediatric rehabilitation and works with families to ensure that the best possible care is carried through from hospital, to rehabilitation, to home. Our nursing staff will provide hands-on training in wound dressing, gastrointestinal tube feeding, and medication management, as well as demonstrate the proper usage of advanced technical equipment that families need to comfortably support their child. Staff works with each family to develop an individual transition plan to move the child from hospital to home.
