Shared Living
Seven Hills Family Services Shared Living Providers welcome adults with developmental disabilities into their homes and lives. Shared Living Providers insure the stability, safety, health, and welfare of those individuals under their care. Shared Living arrangements provide care in a manner that maximizes the individual’s dignity and quality of life and most closely replicates a private home experience. Providers are professionally trained, compassionate caregivers who are well matched with the individual in their care.
Shared Living Providers insure the stability, safety, health, and welfare of the individuals with disabilities that they share their homes with. They also help them to acquire the necessary skills commonly needed at home and in the community. The Shared Living Provider assists with direct care in a manner that maximizes the individual’s dignity and quality of life. Shared Living Providers assist adults with developmental disabilities by providing direct care, support, and community inclusion. The Shared Living Provider is always able to maintain a stable, safe, healthy, and clean environment for the individual in their care. To insure the individuals overall well-being, the Shared Living Provider will work collaboratively with all concerned principals.
Shared Living with Seven Hills Family Services includes:
- 24 hour Case Management
- Financial Supervision
- AFC (Adult Foster Care) Medical and Health Oversight
- Human Rights Advocacy
- Confidentiality Assurance
- Utilizes the DMR Model for Assessment of Services
- Manage and Review for all Sub-Contractor Services
- Extensive Training of all Shared Living Providers
- Family Support
- Community Integration
- Ongoing and Active Management of ISP Goals
