Case Management/Outreach Services

Case Managers provide information and assist HIV+ clients in obtaining a broad range of services including: locating adequate housing; client advocacy for retirement and disability benefits; accessing healthcare; assistance with doctor, hospital and home visits; and translation, interpretation and referral services.  Case Managers are trained to assist with the translation of doctor’s notes, explain medical dosages, treatments, diets and medical follow up care.  Case Managers provide interpretive services and advocacy to Spanish and Portuguese clients.
