Shared Living

Seven Hills Family Services Shared Living Providers welcome adults with developmental disabilities into their homes and lives. Shared Living Providers ensure the stability, safety, health, and welfare of those individuals under their care. Shared Living arrangements provide care in a manner that maximizes the individual’s dignity and quality of life and most closely replicate a private home experience. Providers are professionally trained, compassionate caregivers who are well matched with the individual in their care.

Shared Living with Seven Hills Family Services includes:

  • 24-hour Case Management
  • Financial Supervision
  • AFC (Adult Foster Care) Medical and Health Oversight
  • Human Rights Advocacy
  • Confidentiality Assurance
  • Utilization of the DDS Model for Assessment of Services
  • Management and Review of all Sub-Contractor Services
  • Extensive Training of all Shared Living Providers
  • Family Support
  • Community Integration
  • Ongoing and Active Management of ISP Goals
